Nizhoni Health is a home healthcare organization, providing nursing, home health aides, and physical therapy, among other services across Massachusetts.
With 20,000+ weekly home visits, a network of over 700 nurses, and more than 1,100 referring partners, Nizhoni Health was struggling with siloed patient data and a less than optimal provider referral management process. The visiting nurse company needed a solution to improve its referral management as well as a 360-degree view of its provider relationships. Key details, such as relationship history, important contacts, and referral data were difficult to view.
Strategy & Solution
Ciberspring implemented Salesforce Health Cloud for Nizhoni to create a holistic provider referral management system (PRM). The implementation featured a data integration with Box, leveraged Bridge Connector for clinical integration, and integrated their phone system. These integrations enhanced data integrity and improved the user experience.
The result was increased visibility into their provider relationships and reports with actionable insights to improve their referral process.
